Registration Guidelines

Regarding the online process for registration and renewal of voluntary organizations:

1- Online Process For Registration)

• Online application will be made by the voluntary organization by logging on to the web portal set by the department for registration/renewal by voluntary organizations under the registration scheme of institutions for persons with disabilities in Uttar Pradesh.
• The process of online application can be conducted in all types of internet access devices as well as at Jan Suvidha Kendras (Common Service Centres), Lokvani, Cyber ,Cafes, Private Internet Centers etc.
• While applying online, all the following records/forms/documents related to registration/renewal will be uploaded by the voluntary organization by filling it at the designated place in the application form.

2- Records to be uploaded regarding registration

• Prescribed Application Form
• Memorandum / Trust Deed of the Society, Indian Red Cross Society or any branch
• Manual of the Society
• Archival evidence of whether the institution is registered under the Indian Societies Registration Act 1860 (Act No. 81 of 1860 or under any other law for the time being in force in the State
• Proof of audited details and details of grants received in the last 3 years prior to the date of application
• Details related to the total number of persons employed in the organization and their evidence of duty
• Evidence of the number of professional persons employed in the organization
• Details related to the qualifications of the professional persons employed by the institution and the applicant's residence certificate
• Certificate of having been working in the field of rehabilitation of persons with disabilities for not more than 3 years immediately before the date of application of the institution
• The organization shall submit the attested photograph of the members/officers operating the finance and bank account of the institution along with the application for registration.
• The institution has employed persons registered with the Rehabilitation Council of India to meet the special needs of children with disabilities.
• Certificate that the institution has adequate training and teaching material for the disabled
• The institution has submitted its audited accounts and annual report for the last 3 years. Has not been blacklisted through Chairman or Secretary / Chief Executive Officer (by whatever name called) or Treasurer.
• Affidavit given by the institution that the State and Central Government or designated apex bodies such as National Trust, Rehabilitation Council of India etc. will provide high quality facilities and services to the persons with disabilities as per the standards as prescribed from time to time.

2. Declaration in the prescribed format on a stamp paper of Rs.10 attested by a notary.

3. Action Plan (Details of the work the organization wants to do)

4. Certificate of financial resource (from where the money is being earned by the institution)

5. Records related to whether the building is on rent or operated in a private building.

6. Proof of address of all the office bearers of the institution.

7. It must be mentioned in the rules of the organization to do work for the handicapped.

Register Renewal Guidelines

Regarding the online process for renewal of voluntary organizations:

1.- Online Process (For Renewal)

• The details related to the voluntary institution already registered in the department and the copy of the pre-issued registration certificate will be uploaded on the renewal prescribed portal for verification by the concerned District Disabled Persons Empowerment Officer.

• After verification by the District Disabled Persons Empowerment Officer, if found correct, the username and password will be provided while registering.
• After receiving the username and password by the voluntary institution, online application will be submitted on the login of District Disabled Persons Empowerment Officer of the concerned district, uploading the prescribed online application form and necessary forms to renew the registration certificate by login to the renewal portal.
• The application form received at the login of District Disability Empowerment Officer along with enclosures will be forwarded to the District Disabled Empowerment Officer after verification after verification, will forward the application form to the Directorates login for renewal of registration certificate or return to the login of the voluntary institution if an error is found in the application form. will be dispatched
• The applications forwarded on the login of Directorate of Empowerment of Persons with Disabilities (with their attachments) will be examined by the District Disabled Empowerment Officer. In case the application form is found to be correct after the test, an online registration renewal certificate will be issued, which the voluntary institution can download on its login.
• In case of any kind of error/incomplete found in the application form in the test, the Directorate will be sent back to the District Disabled Empowerment Officer of the concerned district to complete the application form, which will be sent back to the concerned institution login by the District Disability Empowerment Officer for the completion.
